The check-in is open to everyone, regardless of whether they receive services from Communicare. The organization says responses will help inform efforts to better support the community. Support services are always available, and residents are urged to reach out if they need help.
Individuals seeking immediate assistance can call or text 988 to reach the Suicide & Crisis Lifeline, or contact the local 24-hour crisis line at 866-837-7521. A QR code and the assessment form are available online for those who wish to participate anonymously or share their responses digitally, according to Communicare.
